Cord Blood Donor Registration
In order to donate your baby's cord blood, there are a few steps to complete before and after your baby is born. For your convenience, printable forms in PDF format are available here for downloading. We will also be happy to mail you a set of the forms.

Before Your Baby is Born:
1) Call Indiana Cord Blood Bank at 800.804.6703 sometime after your first trimester so we can send you the cord blood collection kit and answer any questions you may have.
2) If you are planning to deliver at a Clarian Hospital, there is an additional consent form you will need to print, sign, and mail to us. Then, a few days later (after we have received the signed consent form), you will need to call us to verify your consent.

3) Fill in the Family Medical History Questionnaire, the Maternal Questionnaire, and the Confidential Maternal Demographic Information forms. You can mail these forms to us before delivery or bring them with you to the delivery and send them along with the collection kit.
4) Make sure your caregivers know that you want to donate your baby's cord blood.
At The Delivery:
1) Bring the cord blood collection kit and the printed forms with you, and make sure your caregivers know that you want to donate your baby's cord blood.
2) Your caregivers will need to fill out the Delivery Information Form and return it along with the cord blood collection kit.
Two Weeks After Delivery:
1) Complete the Follow Up Information Form and mail it back to us. We will send you this form and a pre-addressed and stamped security envelope when we send you the collection kit.
